Pleasant Prairie, WI – Hospice Alliance is pleased to team up with area health and human service organizations on April 16th for a special National Healthcare Decisions Day community event. As part of the 8th annual national event, Hospice Alliance will offer education and information on the importance of completing a Wisconsin Power of Attorney (POA) for Healthcare from 9:00am to 4:00pm at its office located at 10220 Prairie Ridge Blvd., Pleasant Prairie, and WI. Attendees do not need to schedule an appointment.

A short informational video will run, featuring local attorney, Carl Holborn, from O’Neil, Cannon, Hollman, Dejon, and Laing, a Milwaukee firm. Using easy-to-understand language and real-world examples, Holborn outlines the importance of having a POA for Healthcare. Hospice Alliance staff and volunteers will also be on hand throughout the day to assist with questions and completion of the Wisconsin POA for Healthcare form.

“We are pleased to be a part of this very important national initiative,” said Rita Hagen, Executive Director, Hospice Alliance. “Documenting how you would like to be cared for when you are unable to speak for yourself, is one of the greatest gifts that you can give to your family, whether you are young or old.”
