Hours: 8am to 4:30pm

GENERAL DESCRIPTION

Summary

Hospice Alliance is currently seeking a compassionate Visit LPN (Licensed Practical Nurse) to work in a weekend rotation from 8am to 4:30pm. You may choose a rotation of every weekend, every other weekend or every third weekend. The availability to work additional hours on a PRN basis is also desired.

The visit nurse will provide nursing care within the visit nurse’s scope of practice. Visits made by the visit nurse are directed and guided by the RN responsible for leading the shift. Visit nurses travel to the patient’s residence to conduct the nursing visit.

Physical Demands

Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.

Work Environment

Staff may be exposed to virus, disease and infection from patients and specimen’s in
working environment. Staff will be required to travel to and work in patient’s homes and be responsible for own transportation.

ESSENTIAL DUTIES AND RESPONSIBILITES

  • Provide nursing care under the direction of the RN in accordance with the patient’s plan of care.
  • Documents all patient information in an accurate and timely manner.
  • Makes accurate observations of patients and institutes interventions within the scope of practice. Reports significant changes in the patient condition.
  • Administers medications and treatments as ordered by the physician within his or her scope of practice.
  • Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family.
  • Identify and address comfort care needs.
  • Support, instruct and educate the patient, family, and caregiver.
  • Maintain the dignity of the dying patient and support the patient’s and family’s unique spiritual and cultural beliefs.
  • Assists within his or her scope of practice with the immediate after death needs of the patient and family, including calling the funeral home, coroner or other personnel and completion of documentation.
  • When there is no continuous care or other visits to be made, the LPN/RN will assist with patient care in the hospice house or other duties as directed. An RN visit nurse may be assigned to assist in the admissions department.
  • Other duties as assigned

EDUCATION AND/OR EXPERIENCE

  • Graduate of an accredited school of nursing.
  • Valid license as a Licensed Practical Nurse or Registered Nurse in the State of Wisconsin.
  • Hospice Certification and/or prior Hospice experience is preferred.
  • Minimum of one-year recent professional nursing experience.
  • Valid Driver’s License.
  • Computer skills including Microsoft word and e-mail.
